Gundersen Health System is looking for a full-time Nurse Practitioner or Physician Assistant to join a multi-disciplinary Patient Centered Medical Home model in Tomah, which integrates family medicine, internal medicine and pediatrics. This is a .9 FTE, outpatient only position with no call working between the hours of 7:00am to 5:30pm, Monday through Friday.
The current Tomah Medical Staff includes a one full-time and two part-time family physicians, a full-time general internist, a full time pediatrician, a full time general surgeon, a full-time general surgeon physician assistant, a full-time obstetrician/gynecology surgeon, a full-time orthopedic surgeon, a full-time orthopedic physician assistant, two full-time podiatrists, two certified nurse midwife, one family practice physician assistant and two family nurse practitioners. In addition, the Tomah Clinic also provides outreach clinics for cardiology, pediatric cardiology, dermatology, nephrology, podiatry, urology and vascular surgery.
The Gundersen Tomah Clinic practices state-of-the art medicine and connects you with the entire Gundersen Health System with its digital radiology, telemedicine and a system-wide integrated electronic medical record (Epic.) The clinic has full lab and imaging services, including digital mammography. Gundersen Health System has long been committed to medical education and public service. The Tomah Clinic serves as a site for the University of Wisconsin and Wisconsin Academy for Rural Medicine (WARM) third and fourth year students.
