Provides pastoral and spiritual care to patients/families/employees and performs or arranges for religious services/resources to meet the needs of the patients. Functions as a pastoral caregiver in carrying out the programs and ministry of the department, the organization's mission and strategic direction. May serve as a liaison and colleague with parish pastors to coordinate efforts in ministering to the special needs of the patients, participating in education and outreach on spirituality and grief and development of interdisciplinary patient plan of care.
Works with department members and the health care team to provide quality, direct interfaith, clinical ministry to patients, families and staff to meet the standards of practice within the department and/or clinical service area assigned. May take overnight/weekend call and be accessible/available for emergencies.
Participates in interdisciplinary team meetings and rounds, serving on institutional committees and promoting an interdisciplinary approach to spiritual care. Be available for consultation to staff and other individuals on matters pertaining to organization-wide spiritual care.
Will assist with mentoring and coordination of CPE students’ work in order to provide effective support and learning opportunities for the student as set forth by the ACPE Standards. May provide educational opportunities, both individually and in a group setting to patients and those who serve patients/families.
Facilitates conversation and provides education to patients and family regarding Advanced Care Planning.
Maintain required records, reports, statistics, etc. Ensure required documentation on electronic medical records.
For Spiritual Care Department, the chaplain must be certified with a recognized national agency within 3 years of hire date. The following national agencies are recognized:

Theology from an accredited institution or equivalent and ordained or commissioned to function in ministry by an appropriate religious authority and faith group endorsement for chaplaincy and four quarters of Clinical Pastoral Education (CPE) in an accredited ACPE center (s)
