Gundersen Health System works in partnership with a wide range of area organizations to help improve health in the communities we serve.

Assessing health needs is an important part of the mission of hospitals and county health departments. All partners involved in this community health needs assessment process agree their organizations share a common vision of improving community health and yet may prioritize needs and address them differently in subsequent action plans for their organizations.

Community Health Needs Assessment

Through focus groups, we have learned more about the health needs of northeast Iowa. The process, called a Community Health Needs Assessment, is designed to gather information on unmet health needs in the area and identify ways we can help meet those needs.

Our Community Health Needs Assessment identified three priority categories of concern:

  • Healthy Behaviors and Lifestyles
  • Access to Health Care
  • Risky Behavior Education (Alcohol and Drug Abuse and Bullying)

Activities to address identified community health needs (above) are outlined in a report called a Health Improvement Plan. These activities will occur during a three-year period (2017-2019).
